Registration Information

Important Information for participants
· Conference is scheduled on October 31, November 2 to 3, 2009, 9am to 6pm. Venue is the Higher Rock
  Christian Church, 3F ESNA Bldg. #30 Timog Ave., Laging Handa, Quezon City. Please be at venue 8am on
  October 31, 2009 to pick up name tags and conference kits.
· Conference is open to all pastors, elders, church leaders, and workers.
· Registration fee is P600 per person. This includes attendance in all sessions, hand-outs, morning and
  afternoon snacks and lunch.
· Registration deadline is on October 28, 2009.
· Slots are limited and only those with paid registration will be processed.
· Paid fees are non-refundable but transferable.
· Out of town participants are encouraged to make their own prior arrangements for accommodations during
  the conference and travel to and from the venue.

· Register by submitting your completed form through mail, fax or e-mail. For more than one participant,
  kindly make copies of this blank form and submit one form per person.
· Pay your fees by depositing cash or check to our bank account or by visiting us at the Secretariat office.
  Send a copy of your bank-validated deposit slips by mail, fax or e-mail.
· You will receive confirmation of your registration by e-mail or text.
· Please do not send cash by mail.
